Author Biography

Robert Wachter, MD is professor and Chair of the Department of Medicine at the University of California, San Francisco. He is generally considered to be the "Father of Patient Safety" and in 2015, Modern Healthcare named him the most influential physician-executive in the United States.

Kiran Gupta, MD, MPH is an Assistant Professor of Medicine at the University of California, San Francisco.
